A Clinical Study of Traumatic Hemoperitoneum



Abstract
We reviewed and analyzed 67 patients sustaining traumatic hemoperitoneum who were treated at the Seoul Red Cross Hospital between January 1985 and September 1991
@ES The results were as follows:
@EN 1) Abdominal trauma occured in male and younger age group with predominantly high incidence, namely, male to femaleratio was 5.1:1
2) In age distribution, second, third and fourth decades were prevalent age groups(70.1%).
3) In seasonal distribution, winter(32.8%) and spring(29.9%) revealed high incidence.
4) Among 67 cases of the traumatic hemoperitoneum, blunt trauma was 49 cases(73.1%) and penetrating trauma was 18 cases(26.9%), the ratio, approximately 2.7: 1. the causes of injury were traffic accident in 27 cases(40.3%), stab wound in 17
cases(25.4%), and blow or kick in 12 cases(17.9%)in order of frequency.
5) The common clinical manifestations were abdominal pain(98.5%), abdominal tenderness (97.0%), rebound tenderness(92.5%) and abdominal rigidity(61.2%).
6) Among 67 cases of the traumatic hemoperitoneum, paracentesis was performed in 57 cases(85.1%). The result was as follows; positive in 53 cases(93.0%), and false negative in 4 cases(7.0%).
7) 39 patients(58.2%) were operated within 12 hours from the time of injury.
8) The common intraabdominal organs injured were retroperitoneum(28.4%), mesentery and omentum(23.9%), liver(22.4%) and spleen(19.4%).
9) The associated extraabdominal organ injury were chest in 10 cases(14.9%), upper and lower extremities in 8 cases(11.9%) and pelvis in 3 cases(4.5%).
10) The incidence of postoperative complications was 31.3% and common complications were wound infection(14.9%) and intestinal obstruction(9.0%).
11) The overall mortality rate was 7.5%(5 cases) The most frequent cause of death was hypovolemic shock(60.0%) and other causes of death included ARF with sepsis and ARDS.
